Liability Waiver & Release of Risk

ASSUMPTION OF RISK, WAIVER & RELEASE OF LIABILITY

I understand that participation in WNFC National Tryout Day and related football activities involves inherent risks, including but not limited to physical contact, falls, collisions, overexertion, equipment-related injuries, serious bodily injury, permanent disability, paralysis, or death.

By registering and participating, I voluntarily assume all risks associated with these activities, whether known or unknown.

To the fullest extent permitted by law, I release, waive, discharge, and agree to hold harmless the Elite Women's Football Alliance of Dallas (EWFAD) the Women’s National Football Conference (WNFC), the event venue, and their respective owners, officers, directors, employees, coaches, contractors, volunteers, sponsors, affiliates, and agents from any claims, demands, damages, losses, or causes of action arising out of or related to my participation in National Tryout Day, including claims resulting from ordinary negligence.

I certify that I am physically able to participate and understand that I am responsible for determining whether I am medically fit to take part in tryout activities. In the event of an emergency, I authorize event personnel to obtain reasonable medical treatment on my behalf if I am unable to provide consent.

By checking the box below, I acknowledge that I have read and understand this waiver, voluntarily accept its terms, and understand that I am giving up certain legal rights.

Photo, Video & Media Release

PHOTO / VIDEO / MEDIA RELEASE

I authorize the Elite Women's Football Alliance of Dallas (EWFAD), the Women’s National Football Conference (WNFC), and their respective affiliates, representatives, partners, sponsors, contractors, and authorized media personnel to photograph, film, record, and otherwise capture my image, likeness, voice, name, and participation in connection with WNFC National Tryout Day and related activities.

I grant EWFAD and the WNFC the right to use, reproduce, edit, publish, distribute, display, and publicly share these photographs, video recordings, audio recordings, and other media for legitimate promotional, marketing, advertising, editorial, social media, website, broadcast, recruiting, and archival purposes, in any media now known or later developed, without additional notice or compensation to me.

I understand that such media may be edited or combined with other content and that I will not have the right to inspect or approve the finished materials before publication.

I release and hold harmless EWFAD, the WNFC, and their authorized representatives from claims arising from the permitted use of my image, likeness, voice, or recorded participation, except as prohibited by law.
